Medical Release Waiver
I certify that I am the parent or legal guardian of my child(ren). I hereby give my permission for any supervisor, coach or other team administrator associated with the Houston Hydra to seek and give appropriate medical attention for our child(ren) in the event of accident, injury, or illness. I will be responsible for all costs associated with any necessary medical attention and/or treatment.
I hereby waive, release, and forever discharge Houston Hydra and associated supervisor, coach or other team administrator from all rights and claims for damages, injury, loss to person or property which may be sustained or occur during participation in Houston Hydra activities, whether or not damages or loss is due to negligence. I hereby acknowledge that my child(ren) is (are) physically fit and capable of participation in all Swim or Polo Team activities.
*The medical information provided during the registration process will be accessed by the swim / Water Polo coaches in the case of an emergency. Fill out the medical portion of your athlete’s registration completely.
Media Waiver
Without expectation of compensation or other remuneration, now or in the future, I hereby give my consent to Houston Hydra, to use my image and likeness and/or any interview statements from me in its publications, advertising, or other media activities (including our website, Facebook, and Instagram). This consent includes but is not limited to:
(a) Permission to interview, film, photograph, or otherwise make a video reproduction of me and/or record my voice.
(b) Permission to use my name.
(c) Permission to video, photograph, in its publications, other print media, social media (including our website, Facebook, and Instagram), or for educational purposes. By checking this consent form as the parent or legal guardian of the registered athlete(s) I hereby consent to and give permission to the above on behalf of such minor child. Liability Waiver
By registering my child(ren) with Houston Hydra, I agree to participate (or allow my child(ren) and family members to participate) in Houston Hydra, and hereby release Houston Hydra, its directors, officers, agents, coaches, and employees from liability for any injury that might occur to myself (or to my child(ren) and family members) while participating in the Houston Hydra program, including travel to and from training sessions, swim meets or other scheduled team activities.
I agree to indemnify and hold harmless the above mentioned organizations and/or individuals, their agents and/or employees, against any and all liability for personal injury, including injuries resulting in death to me, my child(ren) and/or other family members, or damage to my property, the property to my child(ren) and/or other family members, or both, while I (or my child(ren) or family members) participating in the Houston Hydra program.
